Update Parent/Student Contact Information Using the KPBSD Forms Portal
Student contact information updates can be submitted to your child's school by submitting the Student Contact Update in the KPBSD Forms Portal (https://forms.kpbsd.org).
Prerequisites
You must have a KPBSD Forms Portal account to complete this process. If you have not already established a KPBSD Forms Portal account, see Registering a New Forms Portal Account for guidance.
Solution
Step 1
Login to the KPBSD Forms Portal at https://forms.kpbsd.org
Step 2
In the section titled Assigned forms, click the + Add form button

Step 3
From the list of available forms, select the Student Contact Update checkbox, as well as the student(s) that you would like to submit the form for. You may need to scroll down to see the list of students. Click Add form(s) to complete your form selection.

Step 4
Click the Start Selected Forms button to complete the contact update form. Once signed and submitted, your school will be notified that contact updates have been submitted.
