Accounts
Q: Do I need to register for an account?
If you already have a PowerSchool Parent Portal account, you can use the same account to access the Forms portal. If you need a new account, visit the main Forms portal page at https://forms.kpbsd.org and click the Register New Account button to begin the registration process. Q: I have a PowerSchool or Forms portal account, but cannot remember my username?
The Forms portal allows you to supply either the username or the email address used to register for the account. If you cannot remember your username, simply supply your email address in the username field.
Q: I have an account, but do not remember my password. How can I reset it?
Password resets are available on the main Forms portal page (https://forms.kpbsd.org). Select the Reset Password button to begin the process. For detail on this process, visit the solution article Reset KPBSD Forms Portal Password.
Students
Q: I have a new student that is not listed in my student list. How can I add them?
Students are managed via the Manage Students screen. New student(s) can be added to your account using the process described in the Adding a New Student section of the article Associating Students With a KPBSD Forms Account.
Q: I have and existing or former student that is not listed in my student list. How can I add them?
Students are automatically associated with parent accounts through record matching. Most likely, the student you are expecting to see has a different contact phone or email address recorded at the school, and we are unable to automatically determine that they should be associated to your account. You can and and verify additional mobile phone numbers as well as verify additional email addresses to attempt to match students. This process is outlined in the Search for Existing Student(s) section of the article Associating Students With a KPBSD Forms Account. If all else fails, contact your student's school for further assistance.
Q: I still do not see the students I am expecting after verifying my phone number and/or email address. What should I do?
If you have verified your mobile number and email addresses you believe are associated with your student and they are still not listed, kindly reach out to your student's school to verify contact records.
Q: My student shows a status of No for the Forms Eligible column. What does this mean?
Forms associated with students can only be completed by legal guardians, and for students who are eligible to enroll in school. In some situations, you may be associated to a student as a non-guardian role. For assistance with questions related to student association, contact your student's school.
Additionally, students who are no longer of school age may appear as ineligible to complete forms.
No forms will be presented to students who are ineligible for online forms completion.
Forms
Q: I have forms listed I do not want to fill out. How can I remove them?
Some forms are published to all accounts and cannot be removed. For those forms that can be removed, a minus icon will appear to the right of the form name. Clicking this icon will remove the form.
If you cannot remove a form, but are not ready to complete the form, uncheck the box to the left of the form name to remove it from the forms presented during the form submission process.
Q: Can I print a copy of a completed form?
Absolutely! Within the Forms portal, click the Completed Forms icon (checkbox) along the left navigation menu. Any forms previously completed will be listed and can be viewed or printed.
